Case Management Social Worker

About This Role

This role involves organizing and implementing social work services for patients and their families, working within a team to address patient care needs. Key responsibilities include assessing psychosocial support requirements, coordinating discharge planning, and facilitating connections to community resources.

Responsibilities

• Assess patients/families in need of psycho-social support throughout hospitalization and assist with care coordination or community resources referrals
• Advocate for patient self-determination and appropriate level of care
• Identify and arrange discharge plans, including psycho-social, psychological, and practical assistance across the continuum of care
• Actively engage the patient and family in assessment, problem-solving, and referral processes, including transfer and discharge plans
• Notify patient/clinical team of potential referrals and document psycho-social assessments, plans, barriers, changes, and progress in medical records
• Interact with the interdisciplinary team to develop treatment/discharge plans addressing patient’s/family’s psycho-social support
• Coordinate family meetings and assist patients and families in navigating the healthcare system
• Perform functions to address patient’s psycho-social needs with referrals for counseling, community services, protective services, and financial assistance

Requirements

• Bachelors Degree in social work or social welfare per PA Act 68 of 2008
• Less than 1 year relevant experience
• Basic Life Support certification upon hire
• Excellent interpersonal/communications skills
• Basic computer skills
• Able to maintain confidentiality
• Able to act in a professional manner
• Maintains job competency

Qualifications

• Bachelors Degree in social work or social welfare
• Less than 1 year relevant experience.
