About This Role

The Care Transitions Coordinator will assist patients in navigating post-acute care to positively impact patient outcomes and referral source satisfaction. This role involves integrating clinical guidelines and protocols to develop patient-centered transition plans, promoting quality and efficiency in post-acute care delivery, and representing the company in strategic relationships with healthcare providers.

Responsibilities

• Assist patients in navigating post-acute care to create a positive impact on patient outcomes and referral source satisfaction
• Integrate evidence-based clinical guidelines, preventative guidelines, protocols, and other metrics in the development of patient-centered transition plans
• Promote quality and efficiency in the delivery of post-acute care
• Represent area branches in strategic relationships with health systems, hospitals, inpatient facilities, physicians and physician groups, and executive level opportunities

Requirements

• Graduate of an approved school of nursing, therapy or social work
• Licensed in the state where they currently practice
• Two years' demonstrated field experience
• Demonstrated experience and understanding of federal, state, and local laws and regulatory guidelines governing the operations of Medicare certified home health and hospice
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Maintain automobile liability insurance as required by law
• Maintain dependable transportation in good working condition
• Able to safely drive an automobile in all types of weather conditions

Qualifications

• Graduate of an approved school of nursing, therapy or social work
• 2+ years of demonstrated field experience, with 3 years preferred

Nice to Have

• Registered nurse or physical therapist background
• Three years of field experience
• Previous experience in home health or healthcare sales
