Serve as a Care Coordinator to assess needs, link services and resources, and coordinate care for youth and families within the OhioRISE plan, supporting them in achieving health and outcome goals.
Responsibilities
- Assess needs of youth and families/caregivers
- Provide service and resource linkage
- Coordinate care to support health and outcome goals
- Perform community-based tasks, with some tasks done from home
- Work with individuals, parents/guardians, Boundless staff, community members, and other service providers
- Serve as primary point of contact and liaison for support providers
- Schedule meetings, track progress, exchange documentation
- Follow up on needs and appointments
- Report outcomes for individuals and families on caseload
Requirements
- Bachelor’s degree in psychology, social work, or other related field OR at least three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers.
- High School and GED required
- Experience in family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development
- Current High Fidelity Wrap-Around and CANS Assessor training or the ability to complete such within 90 days of hire
- Training in cultural competency or the ability to complete such within 90 days of hire
- Valid Ohio Driver's License with no more than 5 points on driving record
- Valid car insurance
Qualifications
- Bachelor’s degree in psychology, social work, or other related field OR High School Diploma/GED required with a minimum of three years experience.
- At least three years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or a related public sector human services or behavioral healthcare field, providing community-based services to children and youth, and their family or caregivers.
