About This Role

The Embedded Care Coordinator provides on-site support within long-term care facilities, coordinating care for residents and serving as a key liaison between providers, facility staff, residents, and families. This role plays an integral part in the clinical care team, ensuring continuity and patient-centered support.

Responsibilities

  • Coordinate care for long-term care residents using Ennoble Care’s EMR
  • Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions
  • Collaborate closely with Ennoble Care providers within the long-term care setting
  • Serve as a key liaison between providers, facility staff, residents, and families
  • Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner
  • Accurately document patient interactions and care coordination activities
  • Follow established workflows, documentation standards, and escalation protocols
  • Contribute positively to team collaboration and strong facility partnerships

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally via Microsoft Teams, email, phone, and in person
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, in an on-site role in Reston, Virginia area
  • Valid, unencumbered driver’s license
  • Reliable personal vehicle
  • Current auto insurance with proof
  • Ability to pass a background check and drug screen

Qualifications

  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
