About This Role

Serve as an integral, in-person Care Coordinator within long-term care facilities on the East side of Atlanta/Lithonia, supporting residents with care plans, communication, and telehealth, while working alongside Ennoble Care providers.

Responsibilities

  • Maintain a consistent, reliable daily presence within assigned facilities, serving as an embedded member of the care team
  • Engage proactively and regularly with residents to support care plans, follow-ups, and care transitions
  • Serve as a trusted liaison between providers, facility staff, residents, and families
  • Provide timely, accurate clinical updates to Ennoble Care providers for informed decision-making
  • Support routine and urgent vital sign collection and monitor patients experiencing acute symptoms
  • Facilitate enrollment and monitoring in Remote Patient Monitoring (RPM) for eligible patients
  • Identify and engage appropriate patients for Chronic Care Management (CCM) services
  • Coordinate and support urgent telehealth visits for facility-based patients
  • Facilitate clinical onboarding for new facility-based patients and expedite time to first visit
  • Support the onboarding and training of new Embedded Care Coordinators and serve as a clinical mentor

Requirements

  • Legally authorized to work in the United States of America
  • Active, unrestricted clinical certification or licensure
  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
  • Comfortable communicating proactively and professionally via Microsoft Teams, email, phone, and in person
  • Well-developed organizational and time-management skills
  • Strong working knowledge of healthcare terminology and care coordination principles
  • Proficiency with Microsoft 365, including Outlook, Word, and Excel
  • Ability to work on-site, full-time, Monday through Friday, 8:30 a.m. to 5:00 p.m. EST
  • Valid, unencumbered driver’s license
  • Reliable personal vehicle
  • Current auto insurance, with the ability to provide proof
  • Ability to pass a background check and drug screen

Qualifications

  • Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting
