About This Role

This role involves collaborating with a treatment team to implement patient treatment plans. The advocate provides peer support, advocacy, and recovery services to clients, aiming to build trust and encourage their transition from hospital to outpatient services.

Responsibilities

  • Collaborate with the treatment team to facilitate implementation of treatment plans
  • Provide warm hand-off to participants prior to discharge from hospital to outpatient services
  • Provide a variety of peer support, advocacy, and other recovery services to clients and caregivers
  • Develop a relationship of trust with each participant, functioning as a role model, peer support, mentor, advocate, and ally
  • Communicate hope and encouragement to assist participants in developing readiness for discharge from the hospital and return to their community
  • Provide case management service to identified clients while maintaining their wellbeing and program integrity
  • Offer assistance with independent living skills and community adjustment, helping individuals connect to outpatient behavioral health services and community resources

Requirements

  • High School Diploma or equivalent
  • 1-3 years of relevant experience

Qualifications

  • High School Diploma or equivalent
  • 1-3 years of relevant experience

Nice to Have

  • Current Certified Recovery Peer Advocate or Certified Recovery Peer Advocate - Provisional certification
