Weekend Hospice RN Case Manager

About This Role

Provide personalized hospice care to patients and their families as a Registered Nurse, focusing on symptom management and end-of-life support within a homecare setting.

Responsibilities

• Support patients, families, and loved ones through their end-of-life journey
• Perform a comprehensive assessment to establish goals for symptom management and end of life care
• Utilize effective case management strategies
• Participate as a member of an active interdisciplinary team to address patients’ needs holistically
• Develop and maintain a therapeutic relationship through effective communication with patients and their families
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Registered Nurse with Valid CT RN license
• One year of RN experience
• Valid driver’s license
• Reliable transportation
• Computer proficiency

Qualifications

• One year of RN experience; Homecare or Hospice experience preferred.
