About This Role

Provide personalized, autonomous homecare services to patients while utilizing state-of-the-art technology. Facilitate communication and promote self-advocacy to maintain a therapeutic relationship with patients.

Responsibilities

• Provide personalized care to patients in a rewarding environment that promotes autonomy
• Facilitate communication between the patient and the interdisciplinary team
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Licensed Practical Nurse with Valid CT LPN license
• One year of LPN experience
• Valid driver’s license and reliable transportation
• Computer proficiency

Qualifications

• One year of LPN experience; Homecare experience preferred
