About This Role

Provide personalized, skilled nursing care to patients in their homes while facilitating communication between patients and the interdisciplinary team. Educate patients to promote self-advocacy and independence, utilizing state-of-the-art technology for documentation.

Responsibilities

• Facilitate communication between the patient and the interdisciplinary team
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Valid CT LPN license
• One year of LPN experience
• Valid driver’s license
• Reliable transportation
• Computer proficiency

Qualifications

• One year of LPN experience; Homecare experience preferred.
