Weekend Hospice RN Case Manager

About This Role

Provide personalized end-of-life care to hospice patients and their families by performing comprehensive assessments, managing symptoms, and collaborating with an interdisciplinary team. Utilize effective communication and state-of-the-art technology to document skilled services while promoting patient autonomy.

Responsibilities

• Support patients, families, and loved ones through their end-of-life journey
• Perform a comprehensive assessment to establish goals for symptom management and end-of-life care
• Utilize effective case management strategies
• Be a member of an active interdisciplinary team that addresses patients’ needs holistically
• Develop and maintain a therapeutic relationship by utilizing effective communication with patients and their families
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Registered Nurse with Valid CT RN license
• One year of RN experience
• Valid driver’s license and reliable transportation
• Computer proficiency

Qualifications

• One year of RN experience; Homecare or Hospice experience preferred.
