About This Role

This role involves direct supervision and program management for hospice services, ensuring high-quality, cost-effective care and compliance with regulatory standards. It focuses on clinical excellence for end-of-life care and staff development.

Responsibilities

  • Provide staff guidance in making clinical and case management decisions for end-of-life care and symptom management, adhering to Medicare Hospice Benefit guidelines
  • Assure the development of personnel qualifications and policies
  • Identify and facilitate professional development needs and competency of staff through home visits and evaluation of clinical performance
  • Maintain compliance with regulatory agency policies and procedures by guiding staff in cost-effective clinical and case management decisions focused on outcomes
  • Be accountable for team performance in achieving desired clinical and operational performance measures
  • Utilize clinical software to identify education opportunities, optimize service utilization, oversee patient clinical progression, and trend team outcomes
  • Ensure that clinical and functional outcomes are achieved at the highest level possible
  • Resolve patient care problems collaboratively with internal and external customers
  • Oversee daily patient assignments for clinical staff based on client needs and staff competency to maintain team productivity standards
  • Coordinate referrals and ensure timeliness of Start of Care (SOCs) and adjunct service delivery
  • Ensure continuous assessment of patient needs and development, implementation, and updating of individualized care plans
  • Ensure coordination of patient care, including communication with all physicians involved
  • Determine staffing needs and coordinate hiring to meet service demands for assigned geographic areas
  • Hire, train, coach, and evaluate staff, completing personnel evaluations according to agency timelines
  • Review and analyze team and patient metrics consistent with agency initiatives

Requirements

  • Registered Nurse (RN) with a license to practice in Connecticut
  • Diploma in nursing
  • Basic keyboarding skills and ability to manage electronic calendar and email system

Qualifications

  • A Diploma in nursing. Bachelor's Degree in nursing preferred
  • According to current state licensure regulations. Hospice experience with OASIS and case management certification preferred.
