About This Role

Safely transport residents to and from medical appointments, ensuring their well-being throughout the visit and accurately managing their documents and prescriptions. This role is crucial in providing exceptional healthcare and peace of mind to residents.

Responsibilities

  • Assure the safety of each resident when transporting to and from the doctor’s office and/or hospital
  • Accompany each resident on all medical visits and stay with resident(s) until visit is complete
  • Sign in resident upon entrance to medical office
  • Present resident’s documents to office personnel
  • Before leaving doctor’s office, obtain resident’s documents and any new prescriptions
  • Check to make sure doctor has written on the doctor’s order form
  • If follow-up appointment necessary, obtain appointment cards
  • After returning to facility, report to RCM and be prepared to answer questions concerning visit

Requirements

  • Physically able to help ambulatory and non-ambulatory residents transfer to and from wheelchair to vehicle
  • Possess a valid state driver’s license
  • Qualified as a Certified Nurse Assistant (C.N.A.)
  • Clean driving record
  • Clean criminal record
