Responsibilities
- Provide leadership and support to the care team
- Ensure patient/resident needs are met
- Evaluate team performance
- Work directly with care team members to provide compassionate and professional care
Requirements
- Current LPN license by the State Board of Nursing
- Ability to plan, evaluate and coordinate services
- Experience in triaging tasks
- Supervision experience with nursing team members
- Experience coordinating services to provide an optimum resident and patient experience
Nice to Have
- Experience in the long-term care industry
