The Travel Home Health RN will assess patient conditions, implement care plans, and provide education and support to patients and their families within a community health practice setting. This role involves performing admission processes and managing cases with thorough electronic documentation and interdisciplinary communications.
Responsibilities
- Assess patient conditions and implement care plans
- Provide ongoing education and support to patients and their families
- Deliver patient care within organizational philosophy, policy and standards of community health practice
- Perform admission processes and case management responsibilities
- Utilize thorough and timely electronic documentation and interdisciplinary communications
Requirements
- 1 year Certified Home Care experience
- 1 year Case Management Experience
- Ability to travel to cases as assigned
- Massachusetts RN License
- BLS certification
Qualifications
- 1 year Certified Home Care experience and 1 year Case Management Experience required.
