About This Role

Lead the Patient Care Team for VITAS Healthcare's home care services, overseeing patient care, staff management, quality improvement, and customer service. Ensure continuity of care from admission to bereavement for patients.

Responsibilities

  • Direct all patient care services for the team, including volunteer and bereavement, ensuring integration into the interdisciplinary plan of care.
  • Review information on pending and new patient admissions and discharges to assure eligibility and appropriateness criteria are met.
  • Collaborate with team and attending physicians on palliative care measures for pain and symptom management.
  • Assure complete and accurate data and documentation for timely recertification decisions.
  • Monitor the type and level of team services provided to ensure they are appropriate and responsive to patient/family needs.
  • Manage the team schedule and assure coordination of services 24/7.
  • Facilitate team meetings to assure interdisciplinary input into the plan of care.
  • Interview, select, train, supervise, evaluate, and dismiss team staff in conjunction with the Patient Care Administrator.
  • Oversee staff and volunteer schedules, ensuring equitable workload distribution and productivity.
  • Monitor clinical records and team activities to ensure accuracy, completeness, and adherence to regulatory standards.
  • Address and resolve problems/grievances/service failures experienced by patients/families or physicians.
  • Participate in professional, voluntary, or community service organizations.
  • Control and be accountable for productivity, labor, and patient care costs within budget.
  • Approve all bills/invoices related to patient care services.

Requirements

  • Current and valid R.N. License to practice in the state where the VITAS program is located
  • Two years successful supervisory experience or equivalent in a health care organization
  • Reliable transportation with appropriate license and insurance coverage
  • Private telephone in home
  • Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care
  • Basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary health care team, and knowledge of the process of dying and bereavement

Qualifications

  • Bachelor's degree preferred
  • Two years successful supervisory experience or equivalent in a health care organization.
