This role is for a hospice medical social worker responsible for psychosocial evaluations and ongoing counseling for patients and their families during end-of-life care. The social worker will provide emotional support and work within the plan of care to create meaningful experiences.
Responsibilities
- Conduct psychosocial evaluations
- Provide ongoing counseling to patients and families during end-of-life journeys
- Offer emotional support to patients and families
- Work in accordance with the established plan of care
- Create meaningful experiences for patients and their families
Requirements
- Master's degree in Social Work (MSW) from an accredited program
- Current state social worker license
- 2+ years of clinical work experience
- Reliable transportation
- Ability to lift at least 25lbs
Qualifications
- MS degree in social work from an accredited school of social work approved by the Council of Social Work Education
- 2+ years of clinical work experience, preferably in healthcare or hospice
Nice to Have
