Social Worker I

About This Role

The Social Worker provides assistance to patients and their families, addressing personal and environmental challenges to maximize benefits from medical and welfare services. This role involves assessing patient needs, devising therapy programs, and acting as a patient advocate.

Responsibilities

• Perform assessments to obtain information on home environment, family relationships, health history, and patients' personality traits
• Evaluate gathered data for medical plan of treatment, social service programs, and devise pertinent therapy programs
• Assist patients and/or families through conferences to understand, accept and follow medical recommendations and instructions
• Act as patient advocate and participate in conferences with hospital personnel and community agencies
• Provide advisory services on social problems, arrange for discharge or postoperative care at home or in institutions
• Provide and coordinate discharge planning and follow-up to assure continuity of care
• Provide counseling services to alleviate anxieties and fears concerning disabilities, disfiguring illnesses, or an uncertain future
• Furnish advice on financial assistance to patients or families during illnesses
• Perform or assist in studies and research projects concerned with medical social work problems
• Prepare various statistical and follow-up reports for program planning and department performance evaluation
• Participate in planning educational and social programs with community agencies, the hospital, and home health
• Serve as consultant and resource person for other medical team members on social and personal aspects of patients

Requirements

• Bachelors of Social Work Degree from an accredited program
• Licensure as required in state working

Qualifications

• Bachelors of Social Work Degree from a program accredited by Council on Social Work Education
• Entry level. One (1) year casework experience in health institution, child welfare agencies, psychiatric clinics and family counseling services preferred.
