About This Role

Coordinate patient transfers with seamless one-call convenience for referring providers, ensuring clinical needs are matched to facility services and bed capacity. This role requires multitasking, excellent customer service, and the ability to escalate concerns to facilitate streamlined patient transfers.

Responsibilities

  • Promptly answer, evaluate, and prioritize incoming transfer request calls
  • Communicate effectively with referring and accepting providers, transport services, and hospital staff to complete patient transfers
  • Request and coordinate all medical transportation necessary for transfers
  • Ensure appropriate clinical information is gathered and presented to the accepting physician
  • Provide timely communication to referring and accepting providers
  • Document all communication related to each transfer center call thoroughly and accurately
  • Maintain high familiarity with and compliance with established policies and procedures for patient placement and transfer
  • Demonstrate the ability to make accurate real-time decisions regarding patient transfer processes
  • Effectively triage all incoming requests for service according to center policies and guidelines
  • Follow up with referring and receiving providers to gather feedback for a seamless transfer process

Requirements

  • High School Diploma or Equivalent
  • Graduate of an accredited school of Nursing, Paramedic program, or Emergency Medical Technician training program
  • Licensure (State of Kentucky or Compact State): RN/Paramedic/EMT
  • BLS Required
  • A minimum of 3 years of healthcare related experience in an acute care hospital setting, patient care setting, and/or emergency medical transport setting
  • Basic computer literacy/data entry and technology skills
  • Exemplary interpersonal communication and telephone skills
  • Ability to multi-task in a fast paced environment and retain detailed information
  • Knowledge and basic understanding of EMTALA guidelines
  • Critical thinking and problem solving skills
  • Ability to work independently and as a team member
  • Ability to resolve complaints, problems, and issues with tact and service excellence focus
  • Knowledge of patient medical conditions and medical terminology
  • Ability to recognize and react to changing priorities and make timely and accurate judgment decisions
  • Strong attention to detail

Qualifications

  • High School Diploma or Equivalent; Graduate of an accredited school of Nursing, Paramedic program, or Emergency Medical Technician training program

Nice to Have

  • ACLS Preferred
  • Experience in Critical Care/Emergency Department preferred
