Service Coordinator (Mental Health Case Manager)

About This Role

This role involves assessing clients to identify treatment needs, coordinating interdisciplinary team meetings and discharge planning, and ensuring the overall treatment plan implementation for mental health clients.

Responsibilities

• Assess clients to identify individual problem areas necessitating treatment
• Complete assessments
• Coordinate interdisciplinary team meetings
• Coordinate discharge planning efforts among the multidisciplinary treatment team
• Coordinate the overall treatment plan implementation for the client on the unit
• Work closely with nursing staff to ensure continuity of treatment efforts

Requirements

• Two (2) years of full-time experience or part-time equivalent in a mental health setting OR a bachelor’s degree with a major in psychology, social work or behavioral sciences
• Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles
• Familiarity with regulatory standards

Qualifications

• A bachelor’s degree with a major in psychology, social work or behavioral sciences may be substituted for experience
• Two (2) years of full-time experience or part-time equivalent in a mental health setting, or a bachelor's degree in psychology, social work, or behavioral sciences
