This role is for a Service Coordinator (Mental Health Case Manager) who will assess clients, coordinate interdisciplinary team meetings, and manage discharge planning for individuals on their recovery journey in a mental health rehabilitation setting.
Responsibilities
- Assess clients to identify individual problem areas necessitating treatment and complete assessments
- Coordinate interdisciplinary team meetings
- Coordinate discharge planning efforts among the multidisciplinary treatment team
- Coordinate the overall treatment plan implementation for the client on the unit
- Work closely with nursing staff to ensure continuity of treatment efforts
Requirements
- Two (2) years of full-time experience or part-time equivalent in mental health setting OR a bachelor’s degree with a major in psychology, social work or behavioral sciences
- Demonstrated knowledge of DSM-5 and psychosocial rehabilitation and recovery principles
- Familiarity with regulatory standards
Qualifications
- Bachelor’s degree with a major in psychology, social work or behavioral sciences may be substituted for the two (2) years of full-time work experience
- Two (2) years of full-time experience or part-time equivalent in mental health setting
