About This Role

The Service Coordination Specialist for FamilyWorks acts as the main contact for assigned properties, ensuring high-quality and efficient program execution. This role involves assessing resident needs, providing resources, and linking them to services offered by Chicago Housing Authority's contracted service providers.

Responsibilities

  • Serve as the primary point of contact for residents at assigned properties.
  • Assess residents' needs and provide linkages to CHA's contracted Service Providers and programs.
  • Track referrals and resident participation in CMTS.
  • Identify program issues, make recommendations, and implement procedural and program changes.
  • Collect required data for all programming, record in Salesforce, and submit accurate reports.
  • Report child, dependent adult, and elder abuse and neglect according to State law.
  • Maintain a comprehensive list of available community resources for residents.
  • Work outside of established hours (8:30am-5pm Monday-Friday), as needed or assigned.
  • Actively participate as a member of the site team, collaborating with property manager and staff.
  • Plan and coordinate recreational, cultural, and social activities with property operations staff and residents.

Requirements

  • Bachelor’s degree in related field or equivalent experience of 3-5 years.
  • Familiarity with Microsoft Office Suite (Word, PowerPoint, Excel).
  • Ability to work with spreadsheets and large amounts of data.
  • Valid Driver's License.
  • Good verbal and written skills.
  • Excellent interpersonal communication and customer relation skills.

Qualifications

  • Bachelor’s degree in related field or related experience or a combination of education and 3-5 years of related experience.
  • 3-5 years of related experience.

Nice to Have

  • Bilingual proficiency.
