About This Role

The Service Coordination Specialist serves as the primary point of contact for assigned properties, ensuring the effective execution of the FamilyWorks program. This role involves assessing residents' needs, providing resources and linkages to services, and collaborating with various stakeholders.

Responsibilities

  • Serve as point of contact for residents at assigned properties
  • Attend meetings as directed to support housing activity within CHA
  • Follow CHA guidelines for outreach and engagement for identified sites
  • Work collaboratively with internal and external stakeholders and contractors
  • Directly deliver or create opportunities for activities for CHA residents & linkages to FamilyWorks contracted agencies
  • Track referrals & resident’s participation in CMTS
  • Identify program issues to Division leadership and make recommendations for procedural and program changes
  • Support the Family Community Ambassador program in family developments
  • Monitor assigned summer food sites
  • Survey the resident population to identify priority of needs and solicit resident input on service preferences

Requirements

  • Familiarity with Microsoft Office Suite (Word, Power Point, Excel)
  • Ability to work with spreadsheets and large datasets
  • Good verbal and written communication skills
  • Excellent interpersonal communication and customer relation skills
  • Ability to handle and prioritize multiple tasks and meet deadlines

Qualifications

  • Bachelor’s degree in related field or related experience or a combination of education and related experience
  • 3-5 years of related experience or combination of education and experience
