Provide direct nursing care to patients in an Inpatient Acute Rehab setting, implementing and monitoring patient care plans, and coordinating with other disciplines for well-coordinated patient care.
Responsibilities
- Provide direct nursing care in accordance with established policies, procedures and protocols
- Implement and monitor patient care plans
- Monitor, record and communicate patient condition as appropriate
- Serve as a primary coordinator of all disciplines for well-coordinated patient care
- Note and carry out physician and nursing orders
- Assess and coordinate patient's discharge planning needs with members of the healthcare team
Requirements
- BLS Provider obtained within 1 Month (30 days) of hire date
- Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date
- Diploma from an accredited school/college of nursing OR Required professional licensure
Qualifications
- Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire
Nice to Have
- Previous experience in long term care or IPR
- Ability to manage a fast-paced unit with medically complex patients
- Excellent communication and teamwork skills
- Ability to prioritize, multi-task, and critical thinking skills
