About This Role

Provide and coordinate skilled nursing care to home health patients under physician supervision, ensuring competent application of the nursing process. This role involves managing patient care plans, conducting field visits, and maintaining accurate documentation.

Responsibilities

• Provide competent nursing care and interventions to home health patients according to state and industry standards
• Supervise care provided by Home Health Aides and Licensed Practical Nurses (LPNs) assigned to the care team
• Collaborate with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan
• Demonstrate intermediate knowledge of home care and competency in patient care skills
• Conduct field visits to evaluate and initiate start of care, and monitor clinical outcomes
• Utilize accepted company teaching materials in patient and family education
• Assess, monitor, document, and report progress of patient's health and condition using required documents
• Complete documentation timely and accurately at the point of care, adhering to industry standards
• Ensure patients' rights are honored regarding care, treatment, and services, adhering to physician-approved care plans
• Comply with the corporate compliance program and meet agency productivity standards
• Participate in center/agency surveys, mandatory in-services, case conferences, and staff meetings
• Maintain current license, certification, and clinical records to meet federal, state, and agency regulations

Requirements

• Minimum of one (1) year of RN experience
• Current, active, and unrestricted Registered Nurse (RN) licensure in state of practice
• Current CPR certification
• Reliable transportation
• Liability insurance to complete home visits

Qualifications

• Nursing degree from an accredited college or university, or graduate of an accredited/approved RN program
• Minimum of one (1) year of RN experience

Nice to Have

• Bachelor of Science in Nursing (BSN)
• Acute care experience with adult or aging populations
• Home health and/or skilled nursing experience
