The Clinical Nurse Liaison acts as a facilitator and sales professional connecting hospitals and community services with PruittHealth's home health and hospice agencies. This role evaluates patients for admission, provides clinical expertise, and builds business partnerships to drive referrals.
Responsibilities
- Function as a facilitator to hospitals and community services in need of referring to our organization
- Assist in the evaluation of patients to ensure they are appropriate for admission to home health or hospice agency
- Provide clinical expertise by evaluating agencies' ability to meet patient’s clinical needs
- Develop early and appropriate referrals by creating and sustaining business partnerships with facilities and hospitals
- Take responsibility for a regional territory of hospitals and community services
- Take calls for discharge planners in need of assistance, provide on site clinical assessments, and complete discharge paperwork
- Communicate referral needs and facilitate admissions into the Pruitt Health network
- Provide excellent customer service to all referral sources and network of providers
- Represent the organization in community efforts and education seminars
- Develop business relationships by making effective sales contacts and presentations
- Build relationships with PruittHealth Home Health/Hospice staff teams to ensure effective communication with referral sources
- Communicate well with patients/residents/clients and family members
- Review clinical, sales, and professional journals, and internal information
- Identify territory assumptions, key referring physicians, and personnel in facilities
- Develop, implement, and evaluate quarterly and annual territory plans, strategies, and actions to achieve negotiated goals
Requirements
- 2+ years of experience in home health and/or hospice
- Current RN or LPN license
- Fluent with all regulatory agencies, reimbursement systems and levels of care
- Knowledgeable of private insurance, Medicare and Medicaid coverage criteria, rules and regulations and reimbursement methodologies
- Ability to develop clinical knowledge base to support sales activities
- Ability to effectively communicate both verbally and in writing with different disciplines in healthcare
Qualifications
- Two (2) years of college or business school and/or equivalent combination of experience and training. Bachelor’s Degree preferred.
- Two (2) years’ experience in home health and/or hospice. Healthcare sales/marketing, or any equivalent combination of experience and training is preferred.
