About This Role

Provide skilled nursing care to clients in their homes or the Hilltop Residence, ensuring adherence to physician's plans of treatment and maintaining client well-being.

Responsibilities

• Initiate the plan of treatment and necessary revisions on an individual client basis
• Assess client’s physical, emotional, psychological and financial needs
• Perform skilled nursing services to the client in their residence
• Evaluate and update the physician and other caregivers on the client’s needs regularly
• Initiate appropriate preventative and rehabilitative nursing procedures
• Prepare clinical and progress notes on a timely basis
• Counsel client and family in meeting nursing and related needs
• Provide resources for the client and family to make decisions
• Participate in in-services programs and provides in-services as requested to other staff persons
• Maintain continuing education as required by state law
• Supervise paraprofessionals assigned to the patient care coordinator’s clients
• Maintain and foster a good working relationship with clients and their families, staff, co-workers and community persons
• Demonstrate and practice good safety and standard precautions
• Be assigned on-call hours one evening each week, and one weekend each month

Requirements

• Current Minnesota license as a Registered Nurse
• Valid Minnesota Driver’s License
• Reliable transportation to travel to clients’ home
• Proof of car insurance
• Ability to effectively communicate both orally and in writing
