RN Care Coordinator

About This Role

This role coordinates patient care through multidisciplinary collaboration, including discharge planning, to ensure effective use of hospital and community resources and monitor patient outcomes.

Responsibilities

• Coordinate patient care through multidisciplinary collaboration with medical, nursing and ancillary departments
• Assist with the planning of patient care, including discharge planning
• Assure effective use of available hospital and community resources
• Monitor patient outcomes through concurrent utilization review and proactive discharge planning

Requirements

• Currently licensed as a Registered Nurse by the North Carolina Board of Nursing
• BLS certification through American Red Cross or American Heart Association
• Minimum of 2 years of clinical experience in a hospital setting
• Excellent organizational and communication skills
• Must possess full range of body motion to pass a basic FIT test to include bending, stooping, standing and sitting for extended periods of time
• Must be able to lift and carry up to 30 pounds

Qualifications

• Minimum of 2 years of clinical experience in a hospital setting. BSN preferred.
