Provide personalized and autonomous homecare services to patients as a Revisit LPN, facilitating communication and promoting self-advocacy within an interdisciplinary team. This role serves patients in Shelton, Derby, Ansonia, Oxford, and Seymour, CT.
Responsibilities
- Facilitate communication between the patient and the interdisciplinary team
- Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence
- Utilize state-of-the-art technology to document skilled services provided
Requirements
- Licensed Practical Nurse with Valid CT LPN license
- One year of LPN experience
- Valid driver’s license and reliable transportation
- Computer proficiency
Qualifications
- One year of LPN experience
