Registered Nurse RN - PRN

About This Role

Coordinate and oversee all aspects of patients' home care, collaborating with care teams and educating families on patient conditions. This role involves performing physical assessments, managing medications, and supervising other healthcare providers.

Responsibilities

• Coordinate and oversee all aspects of patients' home care
• Collaborate with patient care teams to develop and implement care plans
• Educate families on patient condition, needs and care
• Perform head to toe physical assessments
• Check vital signs and manage medications
• Give injections, IV therapy or tube feedings
• Administer and maintain catheters
• Complete documentation timely and accurately
• Supervise LPNs and HHAs
• Regularly communicate patient progress to the clinical manager, physician and care team

Requirements

• Current RN license, specific to the state(s) you are assigned to work
• Minimum six months home health experience
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance
• Able to assess patient status and identify requirements relative to age specific needs
• Familiarity with general use and functions of the computer

Qualifications

• One year of clinical experience as a registered nurse (RN) or less than 1 year with approval by VP clinical; minimum six months home health experience if applicable
