About This Role

Provide and direct comprehensive nursing care to patients in their homes, collaborating with physicians and other healthcare professionals to ensure continuity of care and optimal patient outcomes. This role involves initiating, developing, implementing, and revising patient care plans in compliance with regulations.

Responsibilities

  • Initiate, develop, implement, and revise the plan of care in collaboration with physicians and other health care professionals
  • Supervise care provided by home health aides and licensed practical/vocational nurses, including instruction and task assignment
  • Provide required supervisory visits
  • Provide high-quality clinical services within scope of practice and infection control standards
  • Complete comprehensive assessments (OASIS) including medication reconciliation accurately and timely
  • Document patient visits per policy and payer requirements, and sync timely
  • Make initial and/or comprehensive nursing evaluation visits, determining home health eligibility and medical necessity
  • Communicate relevant information timely and effectively with agency staff, physicians, patients, and family members
  • Take direction from Clinical Director and Executive Director, completing assigned tasks and required learning timely
  • Assist in the orientation of new agency personnel and serve as a preceptor for staff and students
  • Participate in survey/survey readiness activities and performance improvement plans
  • Participate in on-call and weekend rotation as needed to meet patient needs
  • Adhere to and participate in the agency's utilization management model

Requirements

  • Current and unrestricted RN licensure in state of practice
  • Current CPR certification or ability to complete within 90 days of hire
  • Proven ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
  • Current Driver's License
  • Vehicle insurance
  • Access to a dependable vehicle or public transportation
