This role supports the operations and growth of the Medicaid community-based accountable care model for HNE BeHealthy ACO, ensuring programmatic success across all Care Management teams within Baystate Health.
Responsibilities
- Support the operations and continued growth of the Medicaid community-based accountable care model for HNE BeHealthy ACO
- Ensure programmatic success of all BeHealthy ACO CM teams across the Baystate Health enterprise
- Work collaboratively with clinical managers and other clinical departments to sustain and grow a unified program
- Assist in developing and maintaining an operational business plan and strategy to grow the clinical program
- Implement methods and workflows to operationalize and improve effectiveness and efficiency of CM teams
- Provide ongoing team productivity oversight through data analytics collection, sharing, and education
- Provide supervision, coordination, technical assistance, and consultation on CM program development and operations
- Ensure ongoing communication and education to all ACO BeHealthy CM teams regarding operational success
Requirements
- 2 years of related experience
- Bachelor of Arts degree
- Excellent oral, written, and presentation communication skills
- Excellent interpersonal skills
- Ability to work independently and in a team-oriented, collaborative environment
- Ability to conform to shifting priorities and timelines through analytical and problem-solving capabilities
- Excellent data analytics skills
- Fluent knowledge of utilizing all Microsoft suite of products
