PRN Physical Therapist

About This Role

As a PRN Physical Therapist, you will assess patient care needs, develop and implement treatment plans, and evaluate patient progress in a home health setting. You will coordinate with the healthcare team to provide comprehensive care aimed at improving patient well-being.

Responsibilities

• Evaluate the patient's functional status and physical therapy needs
• Assist the physician in the development of the therapy plan of care
• Provide services within the scope of practice as defined by state laws governing physical therapy
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician
• Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
• Instruct and inform the health care team, patient, and family/caregivers regarding the plan of care, functional limitations, and progress
• Visit patients according to Plan of Treatment and complete progress notes
• Complete all patient evaluations and develop the PT plan of care within state specific guidelines
• Report outcomes of evaluation, goals, and anticipated projected frequency of care

Requirements

• Current Physical Therapy licensure in state of practice
• Current CPR certification
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
