About This Role

The PRN Home Health Registered Nurse provides coordinated skilled nursing care to patients of all age groups in their homes, primarily in the Cottontown/Greenbriar/Portland/Hendersonville area. This role involves collaborating with an interdisciplinary team to achieve patient goals and maintain high-quality patient care under the supervision of a Clinical Manager.

Responsibilities

  • Assesses home care patients, identifying physical, psychosocial, and environmental needs, and documents findings.
  • Completes OASIS, assessment, and visit paperwork according to agency policy, ensuring clinical notes reflect communication and coordination with physicians and other team members.
  • Communicates significant findings, problems, and changes to the Clinical Manager and physician, documenting all communications and interventions.
  • Supervises and provides clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participates in on-call rotation and emergency calls according to agency policy.

Requirements

  • Registered Nurse (RN) license
  • Basic Cardiac Life Support certification (within 30 days of hire)

Qualifications

  • Associate Degree, or Registered Nurse Diploma
