About This Role

As a Primary Care Nurse (RN) at Hartford HealthCare at Home, you will provide personalized care to patients in a home setting, utilizing your education and training to conduct comprehensive assessments and facilitate communication within the interdisciplinary team. This role promotes autonomy and supports patient self-advocacy and independence.

Responsibilities

• Perform comprehensive assessments to establish an individualized care plan
• Facilitate communication between the patient and the interdisciplinary team
• Develop and maintain a therapeutic relationship by providing education and promoting self-advocacy and independence
• Utilize state-of-the-art technology to document skilled services provided

Requirements

• Valid CT RN license
• One year of RN experience
• Valid driver’s license
• Reliable transportation
• Computer proficiency

Qualifications

• One year of RN experience; Homecare experience preferred
