About This Role

Provide direct and high-quality nursing care to patients in their homes as a Registered Nurse. Coordinate the total plan of care with other healthcare professionals, ensuring continuity and compliance with regulations and policies.

Responsibilities

• Provide and direct provisions of nursing care to patients in their homes as prescribed by the physician and in compliance with applicable laws, regulations, and agency policies
• Coordinate total plan of care with other health care professionals involved in care
• Help achieve and maintain continuity of patient care by planning and exchanging information with physician, agency personnel, patient, family, and community resources
• Provide high-quality clinical services within scope of practice and infection control standards
• Complete clinical nursing assessments per federal/state program requirements and payer needs
• Ensure patient/client eligibility and medical necessity for services as defined by payer and agency policy
• Develop and revise individualized plans of care/service plans with other community providers
• Ensure plan of care frequency and duration meets patient needs and initiate revisions with physician approval

Requirements

• Current and unrestricted RN licensure in the state of practice
• Current driver's license, vehicle insurance and access to a dependable vehicle or public transportation
• Current CPR Certification or ability to complete within 90 days of hire
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
