Provide high-quality post-acute and transitional care to patients in skilled nursing and rehabilitation facilities as an independent contractor. This role offers flexibility, autonomy, and support from a physician-led organization.
Responsibilities
- Provide in-person medical care to patients in designated post-acute facilities
- Perform assessments, manage treatment plans, and maintain accurate documentation in the EMR
- Collaborate with facility teams to support quality outcomes and smooth transitions of care
- Collaborate as applicable in Trumen’s value-based care and transitional care programs
- Develop strong working relationships with facility and clinical staff
- Serve as a liaison between Trumen and facility teams
- Maintain an on-site presence consistent with your agreed schedule and facility needs
Requirements
- MD or DO with a current, unrestricted Texas medical license
- Board Certified or Board Eligible in Internal Medicine, Family Medicine, or Geriatric Medicine
- Reliable transportation within the Houston area
Qualifications
- MD or DO
- 1+ year of experience in post-acute, geriatric, or SNF care preferred
