Physical Therapist (PT) - Mobile Outpatient

About This Role

Provide mobile outpatient physical therapy services to older adults in their homes and independent living facilities. Assess patient needs, develop individualized rehabilitation programs, and educate patients and caregivers to promote independence and restore function.

Responsibilities

• Provide outpatient rehabilitation services in an in-home setting for older adults
• Evaluate and treat in the patient's functional environment (home or independent living facility)
• Assess the need for, develop, and deliver physical therapy programs
• Identify the need for and make referrals to other disciplines to address comprehensive patient needs
• Implement individualized therapy programs designed to restore, reinforce, and enhance performance
• Instruct, educate, and train patients and caregivers in skills for promoting independence and productivity
• Direct activities to promote and maintain health
• Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel
• Adhere to department and patient treatment schedule, complete timely clinical and billing documentation
• Screen, examine and evaluate patients, interpret examination findings, and establish a diagnosis and plan of care

Requirements

• Valid Physical Therapy license in the state(s) of practice or proof of license eligibility
• Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field plus a Bachelor's degree in Physical Therapy OR Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
• Valid driver's license or reliable, independent transportation
