Physical Therapist (PT)-Early Intervention

About This Role

Provide physical therapy services to children aged birth to three, including assessment, treatment, and program planning. This role involves direct patient care, documentation, and communication with families and team members.

Responsibilities

• Provide and direct physical therapy services to children aged birth to three
• Conduct evaluations annually and perform six-month reviews of IFSPs to determine need for intervention and produce goals
• Produce discharge reports in advance of reaching age-out of Early Intervention to assist with the transition process
• Interpret and communicate evaluation findings to parents and other treatment team members
• Establish treatment schedule and provide frequency of services as determined by IFSP plan
• Communicate and document changes to IFSP as needed
• Participate in co-treatment or IFSP consultation with other treatment team members to ensure coordination of patient care
• Develop appropriate home or community programming to maintain and enhance child development
• Document results of assessment, treatment, follow-up, and termination of services
• Adapt treatment plans to meet the individual needs of the child

Requirements

• Appropriate State License

• Willingness to work with medically fragile (for some preschool referrals)
