Deliver in-home outpatient physical therapy services to older adults, focusing on rehabilitation and wellness to restore function and promote independence. Assess needs, develop treatment plans, and provide direct care.
Responsibilities
- Provide in-home outpatient rehabilitation services for the older adult population
- Assess the need for, develop, and deliver physical therapy programs
- Identify the need for and make referrals to other disciplines
- Implement individualized therapy programs to restore, reinforce, and enhance performance
- Instruct, educate, and train patients and caregivers in skills essential for promoting independence
- Direct activities to promote and maintain health
- Supervise Physical Therapy Assistants, temporary licensees, students, and support personnel
- Adhere to department and patient treatment schedule, and complete timely clinical and billing documentation
- Screen, examine, and evaluate patients, including history, systems review, and application of appropriate tests and measures
- Develop appropriate treatment goals and methods collaboratively with the patient and caregivers
Requirements
- Valid Physical Therapy license in the state(s) of practice, or proof of license eligibility
- Master's degree in Physical Therapy OR Master's degree in a Physical Therapy related field in addition to a Bachelor's degree in Physical Therapy OR Bachelor's degree in Physical Therapy plus 5 years of progressively responsible PT experience
- Valid driver's license or reliable, independent transportation
Qualifications
- Master's degree in Physical Therapy, or Master's in a PT related field with Bachelor's in PT, or Bachelor's in PT plus 5 years experience.
