About This Role

This is a temporary, grant-funded Perinatal Community Health Worker position focused on engaging prenatal patients, supporting them with Medicaid enrollment and redetermination, and facilitating connections with resources. The role aims to enhance health literacy and support prenatal and postpartum education.

Responsibilities

  • Engage patients during the prenatal period
  • Support Medicaid enrollment and redetermination by facilitating connections with KYnectors
  • Navigate patients through the healthcare system
  • Identify, manage, and link resources based on social determinants of health (SDoH) screenings
  • Build and maintain culturally responsive relationships with patients and the community
  • Serve as a bridge between medical providers and patients to enhance health literacy
  • Support prenatal and postpartum education aligned with evidence-based care

Requirements

  • Adherence to UK College of Medicine professional standards for dress and conduct
  • HIPAA compliance
  • Completion of required orientations
  • Compliance with all STEPS employee policies

Nice to Have

  • Bilingual with medical interpretation certificate
  • Certified community health worker by the state of Kentucky (required within one year of hire)
  • Prenatal Doula/Postpartum Doula experience
