Provide compassionate, evidence-based nursing care to patients with Substance Use Disorder (SUD) in the Bridge Clinic, a transitional clinic focusing on stabilization and connection to long-term community care. Collaborate with the interdisciplinary care team to ensure coordinated and comprehensive patient care transitions.
Responsibilities
- Provide competent, compassionate, evidence-based nursing care utilizing the nursing process and interdisciplinary teamwork
- Actively manage patient flow, including rooming and triaging
- Triage and help identify level of care and intervention needed in cases of substance intoxication or withdrawal
- Expedite appropriate care and treatment of the patient utilizing assessment, critical reasoning and clinical judgment
- Integrate knowledge of each patient’s history and unique circumstances
- Document nursing assessment, interventions, plan and outcome in the patient record
- Assess, triage, provide nursing consultation, and perform follow-up and surveillance of patient status and outcomes using telecommunications technologies
- Create a therapeutic relationship with patient and family
- Provide health education to facilitate patient treatment, including medications for SUD, PrEP, and safer drug use
- Complete toxicology screening for patients
- Assess and medically manage withdrawal from opioids, or refer for higher level of care if indicated
- Provide outpatient pharmacotherapy for addiction, including opioid antagonist or agonist therapy
- Administer injectable SUD pharmacotherapy
- Consult with members of the treatment team to devise an individualized plan for optimal psychosocial treatment
Requirements
- Clinical nursing experience (0-1 year required)
- Familiarity with nursing principles and skills for patient care and treatment
- Knowledgeable of care required by respective age groups
- Ability to maintain confidentiality and secure sensitive information
- Knowledge of medical terminology
- Excellent verbal and communication skills
- Ability to accurately screen and triage acute patients
Qualifications
- Bachelor's Degree Nursing preferred
- 0-1 year of clinical nursing experience required, 2 years of related nursing experience strongly preferred
