Outpatient Registered Nurse - RN

About This Role

The Outpatient Registered Nurse (Outpatient RN CAP 1) is an entry-level position within the Clinical Advancement Program, responsible for providing and coordinating clinically competent care for an assigned group of patients in an End Stage Kidney Disease (ESKD) setting. This role involves assessment, planning, intervention, and evaluation of patient care, along with delegation of tasks to direct patient care staff. The nurse contributes to decision-making, teaching, leadership, and quality improvement to enhance patient outcomes and facility operations.

Responsibilities

• Perform ongoing, systematic collection and analysis of patient data pre-, during, and post-hemodialysis treatment, documenting findings and modifying treatment as needed
• Assess, collaborate, and document patient/family's basic learning needs to provide initial and ongoing education
• Direct and provide safe and effective delivery of hemodialysis therapy to assigned patients in collaboration with direct and ancillary patient care staff
• Administer medications as prescribed or in accordance with approved algorithms, documenting medical justification and effectiveness
• Initiate or assist with emergency response measures
• Ensure correct laboratory collection, processing, and shipping procedures are performed
• Identify expected outcomes, document, and update the nursing assessment and plan of care through interdisciplinary team collaboration
• Ensure patient awareness related to transplant and treatment modality options
• Complete CAP requirements to advance

Requirements

• Current appropriate state licensure (RN)
• CPR BLS Certification
• Meet practice requirements in the state of employment

Qualifications

• Graduate of an accredited School of Nursing
• Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse.

Nice to Have

• Chronic/acute hemodialysis experience
