Outpatient Registered Nurse - RN

About This Role

This Outpatient Registered Nurse (RN) position provides and coordinates clinically competent care for assigned hemodialysis patients, including assessment, planning, intervention, and evaluation. The role involves participating in decision-making, teaching, and quality improvement activities within the End Stage Kidney Disease (ESKD) healthcare team to enhance patient care outcomes and facility operations.

Responsibilities

• Perform ongoing, systematic collection and analysis of patient data pre, during, and post hemodialysis treatment for assigned patients, documenting in the medical record.
• Assess, collaborate, and document patient/family’s basic learning needs to provide initial and ongoing education.
• Direct and provide, in collaboration with direct and ancillary patient care staff, all aspects of safe and effective hemodialysis therapy delivery.
• Administer medications as prescribed or in accordance with approved algorithm(s), documenting appropriate medical justification and effectiveness.
• Initiate or assist with emergency response measures.
• Serve as a resource for the healthcare team and participate in staff training and orientation of new staff.
• Ensure correct laboratory collection, processing, and shipping procedures are performed and reschedule missed or insufficient laboratory collections.
• Identify expected outcomes, document, and update the nursing assessment and plan of care in collaboration with the Interdisciplinary Team.
• Ensure patient awareness related to transplant and treatment modality options.
• Complete CAP requirements to advance.
• Provide oversight of Patient Care Technicians/LPNs/LVNs/RNs as a Team Leader or designated Nurse in Charge after meeting experience requirements.

Requirements

• Graduate of an accredited School of Nursing
• Current appropriate state nursing licensure
• Current or successful completion of CPR BLS Certification
• Meet practice requirements in the state of employment

Qualifications

• Graduate of an accredited School of Nursing
• Entry level for RNs with less than 2 years of Nephrology Nursing experience as a Registered Nurse; minimum of 9 months experience as a RN and 3 months experience in chronic/acute hemodialysis as a RN for team leader responsibilities.
