Nurse Practitioner or Physician Assistant Per Diem

About This Role

Provide comprehensive primary medical care, manage chronic and acute conditions, and coordinate care for Optum members in nursing homes and assisted living facilities as a per diem Nurse Practitioner or Physician Assistant.

Responsibilities

• Deliver cost-effective, quality care to assigned members
• Manage both medical and behavioral, chronic and acute conditions effectively, and in collaboration with a physician or specialty provider
• Perform comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and Medicaid Services (CMS) regulations
• Ensure all diagnoses are ICD10 coded accurately and documented appropriately to support the diagnosis at that visit
• Ensure all quality elements are addressed and documented
• Perform initial, annual, and post-hospitalization medication reviews and reconciliation
• Facilitate agreement and implementation of the member's plan of care by engaging facility staff, families, and physicians
• Evaluate the effectiveness, necessity and efficiency of the plan, making revisions as needed
• Utilize practice guidelines and protocols established by CCM
• Travel between care sites mandatory
• Coordinate care as members transition through different levels of care and care settings
• Monitor the needs of members and families and adjust the plan of care as situations change
• Evaluate plan of care for cost effectiveness while meeting member needs to decrease high costs and unnecessary hospitalizations

Requirements

• Active and unrestricted license in state of practice, or ability to obtain by start date
• Ability to gain a collaborative practice agreement, if applicable
• Current, active DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to complete physical requirements of the job including moving a 30-pound bag, navigating stairs, and variety of dwelling conditions
• Driver's license and access to reliable transportation
• Willingness to travel between facilities daily
• High level of comfort with Advanced Illness and Advance care planning
• Availability to work 24 hours per month, with expectations that 16 of the 24 hours/month could be during off-hours

Qualifications

• Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program (NP) OR Graduate of an accredited Physician Assistant degree program (PA)

Nice to Have

• 1+ years of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficient computer skills including the ability to document medical information with written and electronic medical records
• Flexibility with shifting priorities
