This role is responsible for managing a patient panel and providing primary care, including acute, chronic, and well visits, in both in-office and telehealth settings. It involves continuous learning, collaboration with clinical teams, and utilization of clinical training to perform in-office procedures for a patient-centered healthcare provider.
Responsibilities
- Manage a patient panel with a broad array of patient needs, conducting a mix of acute, chronic, and well visits
- Treat patients in-office or in testing centers and conduct occasional tele-health visits
- Engage in continuous learning during weekly Clinical Rounds and through other modalities
- Collaborate ongoingly with in-office teammates via daily huddles, as well as with virtual clinical teams
- Utilize specific clinical training and perform in-office procedures
Requirements
- Completed an accredited NP or PA program with a national certification
- Current state license in California
- Experience: 2+ years in outpatient primary care setting, OR 1+ year in outpatient primary care setting coupled with a 1+ year primary care fellowship or urgent care setting
Qualifications
- Accredited NP or PA program
- 2+ years as an Advanced Practitioner in an outpatient primary care setting, OR 1+ year in an outpatient primary care setting coupled with 1+ year primary care fellowship or urgent care setting
