About This Role

Join a collaborative clinical team as a Nurse Practitioner or Physician Assistant to provide comprehensive patient assessments and develop personalized, evidence-based care plans in Cleveland, OH.

Responsibilities

• Conduct detailed patient histories and physical exams
• Develop personalized, root-cause-focused care plans
• Educate and counsel patients on next steps in a clear, compassionate way
• Collaborate with the medical director and clinical team
• Document thoroughly using EMR and AI-powered scribe technology
• Provide follow-up care to monitor outcomes and adjust plans as needed

Requirements

• Graduation from an Accredited Physician Assistant or Nurse Practitioner Program
• PA-C/NP Licensed in OH
• Excellent communication and leadership skills
• Commitment to continuous learning
• Passion for innovation in healthcare

Qualifications

• Graduation from an Accredited Physician Assistant or Nurse Practitioner Program
• Prior experience in functional, integrative, hormone therapy or holistic medicine preferred.

Nice to Have

• Experience and/or background in hormone therapy
• Experience and/or background in functional medicine
• Experience in regen med and/or BHRT
