About This Role

This Nurse Practitioner role acts as a palliative medicine consultant, making recommendations for care plans, symptom management, and goals of care in various settings including hospitals, nursing facilities, and homes. The role involves collaborating with staff and attending physicians.

Responsibilities

• Act as palliative medicine consultant in various settings, including hospital, nursing facility and home
• Make recommendations for care plans, symptom management and goals of care
• Work in collaboration with staff and attending physicians within care settings
• Interact with vulnerable patients and families dealing with life limiting or serious illness issues
• Communicate effectively with patients, families, staff, physicians, and insurers
• Assess patients and implement appropriate intervention, care plans, and treatments
• Lead, motivate, and develop others
• Teach nurses, physicians, and other clinical and non-clinical staff
• Apply principles and practices of integrative medicine and health

Requirements

• Successful completion of a Nurse Practitioner program
• Current registration with the Maryland State Board of Examiners of Nurses as a Nurse Practitioner (Adult, Family or Geriatrics specialization)
• National and State Certification for DEA, CDS
• Completion of healthcare provider CPR course
• At least five (5) years of related experience in a healthcare environment
• At least one (1) year related experience in Hospice and/or Palliative care
• Skill in using computer and electronic medical records
• Effective written and oral communication skills
• Analytical skills to assess patients and implement appropriate interventions

Qualifications

• Successful completion of a Nurse Practitioner program; Master’s in Palliative Care preferred
• At least five (5) years of related experience in a healthcare environment. At least one (1) year related experience in Hospice and/or Palliative care.

Nice to Have

• Master’s in Palliative Care
• Acute Care certification (for Inpatient position)
• Certification in Hospice and Palliative Care through HPCC (ACHPN or equivalent)
• Center for Advancing Palliative Care (CAPC) Designation
