About This Role

Manage the clinical care of patients at GBMC Health Partners, providing primary care to individuals of all ages within a community-focused practice.

Responsibilities

• Assume responsibility for the initial evaluation and treatment of patients
• Perform initial history and physical examinations and document appropriately
• Write standard orders and orders related to pre-existing conditions
• Discuss patient findings with attending physicians and nursing staff
• Evaluate patient progress and alter treatment modalities in collaboration with physicians
• Evaluate nursing, medical, social, and educational needs of assigned patients
• Initiate referrals to ancillary departments and community resources
• Write progress notes for each patient visit
• Establish medical diagnoses in compliance with contract
• Perform technical functions as covered by job description and delegation agreement
• Participate actively in health teaching for patients and families

Requirements

• 2-5 years of experience delivering primary care to patients of all ages, infant to elderly
• Certified Nurse Practitioner Program or Physician Assistant Program completion
• Current registration with the Maryland State Board of Examiners of Nurses as a Certified Adult or Family Nurse Practitioner OR Licensed Physician Assistant
• Completion of 'Healthcare Provider' CPR course, yearly

Qualifications

• Certified Nurse Practitioner Program OR Physician Assistant Program
• 2-5 years of experience delivering primary care to patients of all ages
