Nurse Practitioner, Advanced Practice Provider

About This Role

Provide effective and equitable value-based primary care to adults on Medicare, seeing patients independently and collaborating with physicians. Deliver longitudinal care and build meaningful patient relationships to foster trust and connectedness to care plans, working as part of a collaborative care team.

Responsibilities

• Provide exceptional primary care to adults on Medicare
• Conduct office visits for routine and acute medical issues
• Administer Annual wellness visits and health risk assessments with accurate and specific documentation
• Coordinate care with other providers, specialists, testing facilities, and agencies
• Lead population health efforts in coordination with the Care Team
• Assist the care team with phone triage and patient outreach
• Educate patients on their health conditions, care plans, and treatments
• Participate in Oak Street Health promotional activities
• Conduct home visits as needed

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

Nice to Have

• Experience in primary care: internal medicine, geriatrics, or family medicine
• Understands HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and cross-functional collaboration
• Desire to be part of an innovative model focused on empirically-guided population health
• Bilingual proficiency in applicable areas
