Provide effective and equitable value-based primary care to adults on Medicare, seeing patients independently and collaborating with physicians. Deliver longitudinal care and build meaningful patient relationships within an outpatient practice team.
Responsibilities
- Provision of exceptional primary care
- Conduct office visits for routine and acute issues
- Administer Annual Wellness visits and health risk assessments with accurate documentation
- Coordinate care with other providers, specialists, testing facilities, and agencies
- Lead population health efforts in coordination with the Care Team
- Assist the care team with phone triage and outreach
- Educate patients on their health conditions, care plans, and treatments
- Participate in Oak Street Health promotional activities
- Conduct home visits as needed
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification in a relevant NP specialty (Family, Adult-Gerontology Primary Care, Adult, Gerontological)
- Active, non-probationary state Nurse Practitioner license
- Active DEA license
- US Work Authorization
Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
Nice to Have
- Experience in primary care (internal medicine, geriatrics, or family medicine)
- Understands HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
- Passion for teamwork and cross-functional collaboration
- Desires to be part of an innovative model focused on empirically-guided population health
